Is it fungal? bacterial? or ???
It was long postulated that dandruff was attributed to a person’s individual susceptibility, hyperactive sebaceous glands, poor hygiene, the fungal genus Malassezia, or a combination thereof. Dandruff is an omnipresent condition that affects approximately 50% of the world’s population. It can vary in severity based on a multitude of factors, including, but not limited to: age, gender, host physiology, and host demographics to name a few.
For quite some time, dandruff studies were focused on the fungal genus Malassezia-the predominant species of fungi in the cutaneous biome-as they were proposed to be the causative agent of dandruff. As a result, most prescription-strength shampoos used in the treatment of dandruff contain a fungicidal compound called Zinc Pyrithione (ZPT), which acts to inhibit fungal growth through a mechanism of copper toxicity; however, most medicated shampoos and conditioners are ineffective in combating dandruff, which begs the question: “Is dandruff truly caused by a fungus?”
Recent efforts made by Zhijue Xu and his colleagues of Shanghai Jiao Tong University in China have elucidated that the presence of dandruff may be more closely associated to a commune of bacterial species than the once prospective agents, M. restricta and M. globosa, the most clinically significant species of the Malassezia genus. In their studies, published in May of 2016, a total of 363 adults with varying degrees of dandruff were sampled for an analysis of their cutaneous microflora. These fungal and bacterial profiles were compiled using pyrosequencing, which is a method of DNA sequencing that captures the generation of light when a pyrophosphate is released following deoxynucleotide phosphate (dNTP) incorporation by a polymerase.
The results of their findings revealed an alarming degree of similarity in the Malassezia spp. population between subjects with and without dandruff, suggesting that these organisms may not be the causative agent of dandruff after all. Interestingly enough, the results of the study revealed a correlation between dandruff and the composition of the bacterial commune on an individual’s scalp. Comprising this bacterial make-up are most notably Staphylococcus bacteria and Propionibacterium acnes, which both significantly vary depending on whether the subject had dandruff or not.
Additionally, it was discovered that there was a close association between the bacterial population and both water and sebum content on a person’s scalp. Samples from those with dandruff exhibited elevated levels of Staphylococcus bacteria and very low levels of Propionibacterium acnes, while also having dramatically lower water and sebum content on their scalp. These two microbes appear to be in direct competition with one other as the prevailing species appears to be linked with the absence or presence of dandruff. Propionibacterium acnes, a facultative anaerobe-releases free fatty acids through the hydrolysis of triglycerides (sebum) to promote bacterial adherence in addition to producing bacteriocidins to eliminate other species and thus competition. Individuals without dandruff showed the opposite: elevated levels of Propionibacterium acnes and very low levels of Staphylococcus, which also corresponds with greater water and sebum content in their scalps. If Staph is the main cause of the problem, many proposed natural means of treatment may be the best, such as vinegar with olive oil, tea tree oil, lemon juice, or baking soda.
These findings, while revolutionary, also raises other questions: “Did the lack of water and sebum content give rise to a prevailing Staphylococcus population, or did the Staph cause the dry scalp in the first place?” While difficult to answer, the elucidation of these findings now gives reason to discontinue the use of medicated shampoos and look into alternative holistic treatments, which may better treat the condition while also avoiding the unwanted side effects of such shampoos.

Can Ketogenic Diets Improve Gut Health?
The ketogenic diet (KD) has increased in popularity among dieters over the past few decades and remains to be one of the more controversial diets due to its strict requirements. The diet has been used since the 1920s as a therapy for refractory epilepsy and more recently for Glucose Transporter 1 Deficiency Syndrome (GLUT1 DS).
The diet is also under investigation for treating insulin resistance. Initially, KD was used to mimic the metabolism of fasting and was found to have a suppressant effect on epileptic seizures. For two decades, KD was used as a treatment for epilepsy prior to anti-epileptic drug treatments and is still used when patients are unresponsive to those medications.
Due to some of its beneficial effects on weight loss and metabolism, there has also been a major increase in research on KD over the last 15 years, and some of the more intriguing results include its effect on the microbiome. A study published in April 2018 by David Ma et al., in the Scientific Reports Journal showed that a ketogenic diet alters gut microbial diversity and increases pro-vascular microbiota. Microbial diversity, using the Shannon index for alpha diversity which is a measure of within-sample diversity and a synthesis of both the richness and evenness of the microbial community in a sample, was significantly lower in fecal samples from controls relative to the KD mice. It was found that there was an increase in potentially beneficial microorganisms and a decrease in potentially detrimental microorganisms. According to their data, the diet increased the relative abundance of beneficial gut microbiota (Akkermansia muciniphila and Lactobacillus), and reduced that of pro-inflammatory taxa (Desulfovibrio and Turicibacter) (1). Desulfovibrio are sulfate-reducing bacteria associated with inflammatory bowel disease. However, there are conflicting studies such as one conducted in 2017 by Tagliabue, A. et al., that showed a significant increase in Desulfovibrio spp. in patients affected by GLUT1 DS and subjected to KD(2). Due to conflicting results such as these, the subject continues to be studied, but evidence is mounting that organisms such as Desulfovibrio are reduced by the ketogenic diet.
The human body normally relies on carbohydrates for most of its quick energy, but when carbohydrates are reduced to a very low level, glycogen stores are reduced as levels of the hormone insulin decline. This allows fatty acids to be released from fat stores in the body and liver converts some of these fatty acids into the ketone bodies acetone, acetoacetate and beta-hydroxybutyrate, which can be used as fuel by parts of the brain. Due to the strict nutritional requirements to sustain KD (high fat, moderate protein, and low carbohydrates), the growth of some organisms in the gut are favored over the growth of others, especially those that do not ferment carbohydrates and are better adapted to a high fat and low carbohydrate environment. The diversity of the microbiome is therefore decreased and counter-intuitively may lead to some positive effects on health, such as a reduction in inflammation in the gut. Akkermansia muciniphila abundance is inversely associated with obesity and diabetes and is known to have an effect on fat storage, adipose tissue inflammation, and glucose metabolism. KD has been shown to increase potentially beneficial gut microorganisms, including A. muciniphila and bacteria from the genus Lactobacillus. Lactobacillus has been shown in several studies to decrease body weight and fat. The drug Metformin which is prescribed for type 2 diabetics to increase insulin sensitivity has also been reported to increase the abundance of A. muciniphila (3). This shows that lower glucose levels (which are observed in KD) in the body favor the growth of organisms such as A. muciniphila, which further reinforce its beneficial effects.
While there is still much to be learned on the subject, the ketogenic diet may be a practical solution for treating inflammation in the body, reducing body fat, and treating type 2 diabetes. Due to the strict requirements of the diet, remaining committed to its prescribed rules for longer periods of time may be somewhat unsustainable for most individuals. However, it could be a useful tool beyond treating epilepsy that can benefit health and guide users in helping them to understand how to make better food choices.
Ultimately, reducing the consumption of refined sugars, which are in very high amounts and ubiquitous in today’s processed foods, has been shown to significantly benefit human health. The reduction of detrimental microbiota and encouraging the growth of beneficial organisms in the gut may be an added benefit to the diet which also reinforces its overall utility.
